Requesting your FREE postal kit?

Simply follow the link below and fill in your details to receive a FREE self screening kit from the Chlamydia Screening Office. We receive your request via email, all kits are delivered discretely under plain wrap and come complete with full instructions. Screen yourself in the privacy of your own home, fill in your details and post it directly back to the hospital laboratory. If you have any queries when your kit arrives please don't hesitate to contact us.

REMEMBER: These postal kits are only available if you are aged between 16 and 24 years of age and live in Suffolk. If you are aged under 16 we are sorry that we are unable to mail you a postal kit. Please check the SCREENING SITES page to find a clinic near you, or ring the Screening Office on 01473 275228 and speak to a nurse confidentially.
